1.1 Basics and Definitions

What distinguishes mental disorders from mental health? This topic explores the distinction between mental health and illness, emphasizing the importance of clear diagnostic criteria and delving into the study of etiology, the causes and origins of psychological disorders.
Characteristics of Mental Disorders
A significant challenge encountered by professionals in the field of psychopathology is the establishment of a clear definition for psychological disorders. The most comprehensive and up-to-date definition of psychological disorders can be found in the D.S.M-5 (a.p.a, 2013). This definition incorporates multiple essential characteristics and has changed over time.
Table 1 summary: The DSM-5 definition of mental disorders identifies seven core characteristics and conceptual limitations. A mental disorder is defined as a clinically significant behavioral or psychological syndrome occurring in an individual that involves present distress, disability, or a significantly increased risk of suffering. It must represent a manifestation of behavioral, psychological, or biological dysfunction rather than an expectable or culturally sanctioned response to specific life events. Deviant behavior or conflicts between an individual and society are only considered mental disorders if they are symptoms of an underlying dysfunction. Finally, the definition acknowledges that the concept of a mental disorder lacks precise boundaries and a consistent operational definition that covers all situations.
Characteristics
What distinguishes a mental disorder from a state of mental health are three key characteristics: personal distress, violation of social norms, and dysfunctional behavior.
These three key characteristics can be used to differentiate a mental disorder from a “normal” mental state:
1. Personal distress: Personal distress plays a role in various mental disorders. For instance, individuals with a major depressive episode experience suffering due to their depressed mood, difficulties with sleep and concentration, and reduced motivation. These symptoms can significantly impact their ability to function in daily life. However, it is important to note that not all mental disorders involve personal suffering.
Obsessive-compulsive disorder
O.C.D is a mental disorder characterized by recurrent, intrusive thoughts (obsessions) that cause distress and repetitive behaviors or mental acts (compulsions) aimed at reducing anxiety or preventing a feared outcome.
Attention-deficit/hyperactivity disorder
A.D.H.D is a neurodevelopmental disorder characterized by persistent patterns of inattention, impulsivity, and hyperactivity that impact cognitive, social, and behavioral functioning. Individuals with A.D.H.D often struggle with organization, focusing on tasks, and regulating their impulses, leading to difficulties in academic, occupational, and interpersonal activities.
The term refers to the presence or occurrence of disease, illness, or any other health-related condition in a population.
This term refers to the occurrence of death within a population or the number of deaths that occur within a specific period of time. It is a measure of the overall For instance, individuals with antisocial personality disorder may break the law without feeling remorse. Furthermore, suffering can occur without a psychological cause, such as pain during childbirth.
2. Disability and dysfunction: Mental disorders can lead to varying degrees of impairment in one's lifestyle. For example, someone with social phobia may struggle with a new job that requires public speaking in front of unfamiliar people. Difficulties can also arise in personal relationships when individuals with depression withdraw from social interactions during a depressive episode. This characteristic encompasses behavior that is either inappropriate or interferes with the given situation. For example, someone who is afraid of negative judgment from others and consequently avoids an oral examination, has limited social contacts, and regularly consumes alcohol to relax, would be exhibiting inappropriate behavior.
3. Violation of social norms: While this criterion does not apply to all mental disorders, it is still relevant in many cases. An anxious person, for instance, may conform to social norms while suffering from social phobia. However, this criterion is applicable to numerous other disorders, such as the compulsive rituals seen in individuals with obsessive-compulsive disorder (O.C.D) or impulsive behavior in those with attention-deficit/hyperactivity disorder (A.D.H.D). It is important to consider that social norms can vary across societies and cultures.
These three characteristics serve as important guidelines in distinguishing mental disorders from “normal” mental states. As is clear from the descriptions of the three central characteristics of mental disorders, none of them alone can provide a satisfactory definition. Taken together, however, they can provide a framework for defining psychologically deviant experience and behavior.

Epidemiology
Epidemiology investigates how (mental) diseases are distributed in the population and which factors are important for health and illness. It also takes into account morbidity (prevalence/incidence), injuries, impairments and mortality.
Epidemiology in the field of clinical psychology and psychotherapy encompasses several important tasks, including:
1. Determining the frequency of mental disorders: Epidemiology plays a crucial role in assessing the prevalence and incidence rates of mental disorders within populations, providing valuable insights into their occurrence and distribution.
2. Improving their definition (descriptive epidemiology and psychopathology): Epidemiological studies contribute to refining the definition and understanding of mental disorders through descriptive epidemiology and psychopathology. This involves characterizing the symptoms, course, and associated features of these disorders.
3. Assessment of the care system and care needs: Epidemiology helps evaluate the existing care system for mental disorders and assesses the corresponding care needs of individuals affected. This information guides the development and improvement of mental health services.
4. Studying the cause (etiology) and development (genesis) of mental disorders: Epidemiological research investigates the factors contributing to the development and onset of mental disorders, including genetic, environmental, and psychosocial influences. Understanding the etiology and genesis of these disorders is crucial for effective prevention and intervention strategies.
5. Deriving measures related to prevention, therapy, health promotion, and rehabilitation: Epidemiological findings inform the development of measures aimed at preventing mental disorders, promoting mental health, providing effective therapies, and supporting rehabilitation efforts. This involves translating research into actionable strategies that benefit individuals and communities.
These tasks highlight the multifaceted role of epidemiology in clinical psychology and psychotherapy, encompassing various aspects from understanding the frequency and definition of mental disorders to identifying effective interventions and promoting mental well-being.
Epidemiological studies enable the discovery of possible causes and risk factors of mental disorders, while understanding the underlying mechanisms of action requires experimental research designs. Basic epidemiological concepts, such as prevalence, incidence, and risk, are relevant in clinical psychology.
Prevalence describes the frequency of a disease, representing the percentage of all cases of illness (e.g., depressive illness) in a defined population (e.g., persons in the U.S.A aged 18 and over). The prevalence rate is the prevalence at a specific point in time (e.g., on January 1, 2021). Additionally, the 12-month prevalence and the lifetime prevalence refer to the corresponding periods and are often considered when examining the frequency of mental disorders. For example, a 12-month prevalence of 10% would indicate that 10% of the population would have the disorder within one year, while a lifetime prevalence of 10% would mean that 10% of the population would develop the disorder at some point in their lives.
Incidence, on the other hand, refers to the frequency of new cases of a disorder within a specific period (e.g., 12-month incidence). For instance, if the 12-month incidence of bipolar disorder were 5%, it would mean that 5% of the population would develop the disorder within one year. Risk, on the other hand, is the probability of contracting a certain disease by a specific point in time, often estimated based on known incidences and prevalence.

1.2 Paradigms of Clinical Psychology

Different paradigms can be identified within the field of clinical psychology, guiding the specific research questions investigated and the methodologies employed by scientists.
Paradigm
In the context of clinical psychology, it refers to a framework or conceptual model that shapes the way scientists approach research questions and methodologies. It encompasses the underlying assumptions, theories, and perspectives that guide the study of psychological phenomena within the field.
The Biological Paradigm
According to the biological paradigm, mental disorders primarily arise from disrupted biological processes, such as genetic predispositions. Twin studies have demonstrated particularly high heritability for certain disorders.
Another significant aspect of the biological paradigm is the study of the biochemistry of the nervous system. The neuron, the fundamental unit of the nervous system, comprises the cell body with the nucleus, dendrites, axon, branches, and axon terminals. Neurons communicate and transmit information through specific chemical substances called neurotransmitters.
These neurotransmitters are released from the corresponding neuron at the axon terminal and enter the synaptic cleft. From there, they can bind to suitable receptor sites on the cell body of the subsequent (postsynaptic) neuron, exerting their effects by activating or inhibiting other biochemical processes. Once the biochemical signal is transmitted, neurotransmitters that remain in the synaptic cleft are either broken down or reabsorbed in the presynaptic axon terminal through a process known as reuptake.
The following neurotransmitters play a relevant role in mental disorders: norepinephrine, serotonin, dopamine, and gamma-aminobutyric acid gaba. For example, the importance of dopamine was shown in connection with schizophrenia.

Biological explanations play a significant role in understanding and treating mental disorders, particularly through the use of treatment methods such as medication. Various biological factors, such as the stress axis and the immune system, are now recognized as being associated with mental health. However, it is essential to acknowledge that mental processes are highly complex and should not be oversimplified or solely attributed to biological processes (see also the section on the integrative paradigm).

The Psychodynamic Paradigm
According to the psychodynamic approach, unconscious conflicts and developmental disorders (especially in childhood) are seen as the cause of the development of a mental disorder. Sigmund Freud is one of the best-known representatives of this approach. According to Freud, the human psyche consists of three parts: the id, the ego, and the superego. The id represents drives and impulses and is present from birth.
The ego develops from about the sixth month of life and follows the reality principle. Its job is to mediate between external reality and the impulses of the id. The superego does not develop until the third or fourth year of life, when the child is confronted with the (moral) demands and norms of the parents and adopts them.
Black box
This phrase is a metaphor used to describe psychological processes that are inherently subjective and cannot be objectively described, measured, or reproduced. It highlights the elusive nature of these processes and emphasizes their reliance on individual experiences and interpretations.
When experiencing unpleasant sensations (such as fear or discomfort), the ego reacts with unconscious fear-reducing strategies known as defense mechanisms. Repression is considered one of the most significant defense mechanisms, involving the act of keeping unpleasant impulses or desires out of consciousness.
Freud's work has faced significant criticism in recent decades, primarily due to his psychoanalytic theory relying on subjective observations rather than objective scientific criteria. Nonetheless, his work has yielded three assumptions that continue to be acknowledged today:
1. Childhood experiences play a role in shaping adult personality development, with a particular focus on parent-child relationships in contemporary clinical practice.
2. Unconscious processes influence human behavior.
3. The causes and motives behind human actions are not always evident and require a willingness to delve beyond surface appearances.
The Humanistic Paradigm
Therapies based on a humanistic approach assume that the central element in changing a patient is becoming aware of problematic experiences and behavior. A prominent figure in the humanistic principle was psychologist Carl Rogers, who developed client-centered therapy. This form of therapy revolves around three fundamental variables: empathy (empathetic understanding), acceptance (unconditional appreciation), and congruence (authenticity). Empathy, which involves the ability to empathize and take perspectives, serves as a foundational therapeutic attitude aimed at fostering the patient's self-empathy. Acceptance or unconditional appreciation entails accepting the patient as they are without reservation. This does not mean that the patient's behavior is always seen as positive; nonetheless, the patient should be respected and valued as an individual seeking to address their problems. Congruence or authenticity signifies that the therapist should engage in genuine interaction with the client, without assuming a role or putting on a facade.
The Learning Theory Paradigm
At the start of the 20th century, American psychologist John B. Watson brought about a revolution in psychology through his ideas on behaviorism. The core emphasis of this approach is on observable behavior, considering human consciousness as a black box. Within this paradigm, cognitions are regarded as of secondary importance in comparison to behavior.
Three central forms of learning that hold significant importance for the behavioral therapy approach in psychotherapy are classical conditioning, operant conditioning, and model learning. Classical conditioning, initially studied by Ivan Pavlov during his investigations into the digestive system of dogs, involves the association between stimuli and responses. For instance, Pavlov observed that a dog began salivating (conditioned response, C.R upon hearing a bell (conditioned stimulus, C.S that was consistently paired with the presentation of food (unconditioned stimulus, U.C.S. This learned stimulus-response mechanism was extinguished when the bell is repeatedly rung without food. The association of stimuli and reactions also plays a role in the development of mental disorders, with therapy for anxiety disorders often focusing on modifying reactions to phobic stimuli through confrontation and habituation.

Operant conditioning, on the other hand, involves learning based on the consequences of one's own actions. For example, if a child's mother reads them a beloved bedtime story as a positive reinforcement for going to bed on time, the child is likely to repeat this behavior. Similarly, the absence of a negative consequence, such as not receiving a phone ban for adhering to bedtime, can increase the likelihood of a behavior occurring.
Operant learning is particularly relevant in understanding the maintenance of mental disorders, as individuals with anxiety disorders often engage in avoidance behavior to evade negative consequences, such as experiencing fear. Therapy aims to reduce avoidance and facilitate confrontation with feared situations or objects to achieve a habituation effect.
The third form of learning, known as vicarious learning or model learning, was extensively explored by Albert Bandura. Bandura and Menlove (1968), for instance, demonstrated that children who observed other children interacting with dogs without fear gradually reduced their own fear of dogs. Vicarious learning plays a crucial role in acquiring new behaviors and overcoming fears through observation and modeling.
The Cognitive Paradigm
Cognitive processes encompass perception, recognition, understanding, judgment, and reasoning. In contrast to the principles of behaviorism, the cognitive paradigm focuses on how individuals think about and interpret situations. Within this framework, new information is consciously processed and integrated into an organized network of pre-existing knowledge, known as schemata or schemes. Cognitive therapy aims to modify thoughts that are influenced by cognitive distortions or thinking errors. For example, the fallacy of “all or nothing” thinking can lead to thoughts such as, “If I perform poorly on this exam, I am a total failure and should quit the entire course.” Another example is arbitrary reasoning, where a person might believe that a friend's failure to say hello indicates indifference towards them. Albert Ellis developed rational-emotive therapy, which proposes that individuals' thoughts about themselves (e.g., “I should/must do everything perfectly”) are based on irrational and dysfunctional beliefs that can trigger negative emotional responses. According to Ellis, the imposition of unrealistic “should” and “must” demands on oneself and the environment can lead to long-term psychological distress.
The Integrating Paradigm
The paradigms discussed thus far reveal different perspectives on the origins and development of mental disorders, which need not necessarily contradict one another (Kring & Johnson, 2022). Present-day understanding of the etiology of mental disorders follows an integrative approach known as the diathesis-stress model, or vulnerability-stress model. This model proposes that mental disorders arise from the interplay of biological, psychological, social, and environmental factors (Kring & Johnson, 2022). Specific genetic traits, such as those associated with schizophrenia, can be understood as genetically inherited predispositions. Prenatal and perinatal complications can also contribute to the biological diathesis. Psychological diatheses, such as certain personality traits (e.g., neuroticism) and dysfunctional cognitive schemas, play a significant role in the development of depress
Etiology
The study of causes, etiology, plays a crucial role in unraveling the origins and contributing factors behind diseases, aiding in the development of effective prevention and treatment strategies.
Diathesis
Also known as vulnerability, diathesis represents an individual's inherent predisposition to develop a disease. It involves a combination of genetic, biological, and environmental factors that interact to influence disease susceptibility and progression.
Perinatal
This refers to the period encompassing the time immediately before, during, and shortly after birth.
sion and anxiety (Kring & Johnson, 2022). Additionally, social norms can contribute to the development of mental disorders, as seen in the case of eating disorders (Kring & Johnson, 2022).
The presence of a diathesis can increase the risk of developing a particular mental disorder but does not necessarily lead to its onset (Kring & Johnson, 2022). Harmful or unfavorable environmental stimuli, such as critical life events (such as losing a job, separation, or the death of an important caregiver) and chronic stressors, are included in the “stress component” of the integrative approach. According to the integrative approach, both components – the diathesis and the stress component – are important for the development of a mental disorder. In the case of schizophrenia, for example, there is a strong genetic component. Nevertheless, a disorder does not have to manifest itself in people with a corresponding predisposition if the stress level does not exceed the individual stress limit. On the other hand, in other mental disorders, such as post-traumatic stress disorder, the experience of stress plays a greater role than genetic predisposition. Thus, an integrative, multifactorial, bio-psycho-social vulnerability-stress model can be formulated for mental disorders that explains the development of mental disorders in general. This framework model is also used when developing individual disorder models in the context of psychotherapy.

1.3 Classification and Diagnostics

The treatment of individuals with mental health problems necessitates the establishment of an appropriate psychological or psychiatric diagnosis. The process of classification in psychological diagnostics holds significant importance in organizing and understanding complex phenomena. By categorizing phenomena with specific characteristics into structured systems, classification provides a framework for systematic assessment and analysis. It enables clinicians and researchers to identify patterns, make comparisons, and establish a common language for discussing psychological phenomena.
International Classification of Diseases
The I.C.D-11, developed by the World Health Organization (who), is an international classification system for diseases and health conditions, including mental disorders. It provides a standardized framework for the identification, classification, and coding of various mental health conditions, aiding in accurate diagnosis and treatment globally.
The D.S.M-5, which is the most recent and currently utilized edition of the Diagnostic and Statistical Manual of Mental Disorders, is a comprehensive classification system developed by the American Psychiatric Association (a.p.a). It provides clinicians with standardized criteria and terminology for the identification, diagnosis, and treatment of various mental disorders, both in the United States and internationally.
Operationalization
This is the process of defining and measuring variables or concepts in a way that allows them to be objectively observed or measured. It involves specifying the operations or procedures that will be used to assess or quantify a particular construct, making it more measurable and concrete.
Furthermore, diagnostics, which involves the classification of individuals based on specific characteristics, allows for the identification and differentiation of various diagnostic categories within the classification system. This aids in accurate diagnosis, treatment planning, and research on psychological disorders, facilitating effective communication and enhancing the overall understanding of mental health conditions.
Various psychological symptoms, such as a depressive mood, difficulty concentrating, or sleep disorders, often co-occur and can be categorized into a syndrome, such as a depressive syndrome. Based on this syndrome, a diagnosis, such as unipolar depression, can be determined. The criteria for mental disorders can be easily referenced in suitable manuals. There are two main classification systems available for this purpose: the International Classification of Diseases (I.C.D-11), which is published and continuously developed by the World Health Organization (who), and the D.S.M-5, which is updated by the American Psychiatric Association (a.p.a) at irregular intervals. The I.C.D-11 is predominantly used for classification in Europe, while the D.S.M-5 is applied in the United States, among other regions.
The assignment of a mental diagnosis facilitates communication among various healthcare professionals (physicians, psychologists, nursing staff, etcetera) involved in the treatment of individuals with mental illness. However, it is important to acknowledge that the assignment of a mental diagnosis inevitably involves a simplification of information and may not fully account for individual aspects of the patient. The evolution of classification systems throughout the latter half of the 20th century has demonstrated that mental diagnoses should not be regarded as rigid constructs. Notably, the D.S.M considered homosexuality a mental disorder until 1973, while the who removed it from the list of mental disorders almost two decades later, in 1993, with the publication of the I.C.D-10. The classification of mental disorders is influenced by societal changes and prevailing social norms.
Earlier versions of classification systems faced challenges related to the lack of consensus among diagnosticians regarding the assignment of mental disorders. When two diagnosticians arrive at different diagnoses for the same individual, it indicates low interrater reliability. In response, current classification systems are designed with specific criteria that can be objectively assessed and observed by diagnosticians, such as the requirement of at least one episode of binge eating per week over a period of three months for a diagnosis of bulimia nervosa. This approach is known as operationalized and criterion-oriented diagnostics (Rief & Stenzel, 2012).
D.S.M 5 and I.C.D 11
In contemporary psychiatric classification systems, D.S.M-5 (a.p.a, 2013) I.C.D-11 (who, 2018) serve as the primary frameworks for diagnosing mental health conditions. While both systems offer comprehensive guidelines for diagnosing various disorders, they often differ in terminology, criteria, and classification approaches.
D.S.M-5
The D.S.M-5 represents the current version of the D.S.M and was published by the a.p.a in 2013. Unlike the I.C.D, the D.S.M only includes mental disorders. The D.S.M-5 and the I.C.D-10 employ a categorical diagnosis approach, wherein individuals are assessed to determine whether they belong to a specific group or category. To be assigned to a category, a set number of symptoms or criteria must be met. The strong differentiation of mental symptoms is also criticized because almost half of mentally ill people also have another mental disorder (comorbidity). The highly differentiated approach is opposed to the transdiagnostic approach (Harvey et al., 2004), in which psychopathogenic processes are identified that occur across different disorders. The U.S National Institute of Mental Health N.I.M.H is currently developing a theoretical framework that is intended to describe mental disorders based on neurobiological factors and a dimensional classification (Cuthbert & Insel, 2013). While the categorical approach only divides into healthy and sick and can thus lead to the stigmatization of those affected, a dimensional approach could better reflect the severity of mental symptoms and counteract the stigmatization (Kring & Johnson, 2022).
I.C.D-11
The I.C.D-11 is a system developed by the who to classify and code diseases, disorders, and other health conditions. The most recent version was released in 2018, representing a significant update from its predecessor, I.C.D-10. It incorporates advances in medical and scientific knowledge and includes various changes and revisions to the classification of mental disorders. I.C.D-11 (who, 2018, p. 1) defines mental, behavioral and neurodevelopmental disorders as syndromes characterized by clinically significant disturbances in an individual's cognition, emotional regulation, or behaviour that reflects a dysfunction in the psychological, biological, or developmental processes that underlie mental and behavioural functioning. These disturbances are usually associated with distress or impairment in personal, family, social, educational, occupational, or other important areas of functioning.
The development of the I.C.D-11 followed a timeline that began in 2007 with the appointment of an advisory group. Formative field studies were conducted between 2010 and 2011, and in the fall of 2012, the first draft of proposals was posted on the World Health Organization (who) website. Field trials took place from 2012 to 2013, and final changes were made in 2014. In 2015, the I.C.D-11 was approved by the World Health Assembly.
I.C.D-11 brought about significant changes to the classification and structure of diagnostic codes (Gaebel et al., 2020). Unlike its predecessor, I.C.D-10, which had limitations in the number of divisions at each level, I.C.D-11 allowed for an expanded number of divisions by utilizing alphabetic characters or two-digit codes per character. This resulted in more specific groupings of disorders.
I.C.D-11 also aimed to regroup diagnostic classes based on common underlying etiological factors, rather than solely relying on common presenting symptoms (Gaebel et al., 2020). In I.C.D-11, “Disorders Specifically Associated With Stress” were given a separate diagnostic
Comorbidity
This is the co-occurrence of multiple mental disorders in an individual over a specific period of time. grouping, which includes conditions like Post-Traumatic Stress Disorder (P.T.S.D), Complex P.T.S.D, Prolonged Grief Disorder, Adjustment Disorder, and Attachment Disorders (who, 2018).
Regarding diagnostic guidelines, I.C.D-11 provides clinicians with comprehensive guidance for making diagnoses (Gaebel et al., 2020). The guidelines consist of bullet points that describe the essential features of each condition, establish thresholds for normality or minimum impairment levels, and help differentiate the condition from other disorders.

To determine whether an individual meets the diagnostic criteria for a specific disorder, it is recommended to employ a structured interview such as the Structured Clinical Interview. In addition to clinical interviews, mental health professionals have at their disposal a variety of standardized psychological tests and questionnaires. These may include personality tests such as the neo Personality Inventory or the Minnesota Multiphasic Personality Inventory. Cognitive and neuropsychological tests, such as intelligence tests, along with self-report questionnaires like the Beck Depression Inventory, can also be utilized. According to Lazarus (2011) the behavioral variable (B) focuses on observable and measurable open behaviors such as gestures, actions, and reactions. The affect variable (A) encompasses emotions, moods, and strong feelings. Sensations (S) include the five senses: sight, hearing, touch, and smell.
Ideas (I) encompass recurring dreams, vivid memories, and mental images of the present and future. Cognitions (C) refer to a person's thoughts, values, opinions, and attitudes, including self-perceptions and perceptions of the environment (e.g., "I am stupid"). The variable of social relationships (I) reflects an individual's connections with life partners, friends, relatives, colleagues, or superiors. The "Drugs" (D) variable pertains to a person's health or medical condition.
The multimodal model of B.A.S.I.C I.D assumes that these areas influence and interact with each other. Therapeutic interventions can be tailored based on the individual modalities, such as exposure training for specific anxiety disorders.
